1. Do you have the below listed medical conditions:

• Diabetes Mellitus
• Hypertension
• Renal (Kidney) Failure
• High Cholesterol
• History or Current Use of Tobacco Products
• Heart Disease Stroke

2. Do you have a family history of any of the medical conditions described above?

3. Do you experience leg pains with walking or even at rest?

4.  Do you have any wound or cuts to your legs or feet that have taken a long time to heal, or not healed at all?

5. Do you develop leg swelling?

6. Do you have varicose veins?

7. Do you have a family history of Abdominal Aortic Aneurysms?
